How will you CONTRIBUTE and GROW?
You will develop professionally with the following mission:
Supporting people with diabetes in their training process, ensuring they acquire the necessary knowledge for managing their condition, which should translate into better self-care and quality of life.
Responsibilities include:
* Training patients in the use of advanced diabetes technologies.
* Collecting relevant information related to training processes.
* Monitoring patients to verify proper use of technologies.
* Updating patient records continuously.
* Collaborating on the development of diabetes service offerings.
Once the service offering is implemented, additional functions will include diabetic education, patient follow-up, triage, managing alerts from telemonitoring platforms, and applying referral and action protocols according to established procedures.
Are you a MATCH?
You will be suitable if you meet the following requirements:
* University Education:
DUE/Degree in Nursing.
* At least four years of professional nursing experience, preferably in diabetes care.
* Dynamic and proactive personality.
* Geographical mobility within Catalonia and Balearic Islands, as travel to hospitals/centers in these regions is required.
Regular travel is expected.
