Torres Strait Regional Council (TSIRC) is the largest employer in the Torres Strait serving 15 'outer' island communities with unique characteristics and service requirements. About the role:
You will be responsible for delivering in-home and community based aged care services that focus on re-enablement, wellness, and increased independence for consumers. Services will align with each individual's Care Plan, developed in consultation with families and key stakeholders.
Key Responsiblities:
- Monitor and report urgent changes in a consumer's health or environment to the Manager Community Services.
- Maintain accurate and timely documentation, including:
- Daily journal of visits, tasks and observations
- Weekly reports and check-ins with the Manager.
- Encourage consumer and family feedback to continually improve care quality.
- Uphold strict confidentiality and dignity of all consumers.
- Work collaboratively with Indigenous Knowledge Centres (IKCs) and the Healthy Lifestyle Program to support participation in social and community activities.
What We're Looking For

- A genuine passion for working with Elders and supporting community wellbeing.
- Strong communication and interpersonal skills.
- Cultural awareness and a respectful understanding of Ailan Kastom and community protocols.
- Ability to work independently and maintain professional boundaries.
- Basic administrative and reporting skills.
- Minimum one (1) year experience in a similar role.
- National Police Check upon commencement.
- Working with Children (Blue Card) for paid employment.
- Certificate III in Aged Care or related discipline or wiillingness to undertake studies.
- Current First Aid and CPR (or willingness to obtain).
- Queensland Driver's Licence.
- Comprehensive knowledge and understanding of languages specific to the Torres Strait including: Meriam Mer, Kala Lagaw Ya, and Creole would be highly desirable.
