We are seeking a candidate to fill a position in our home care team, providing person-centered support to individuals in dependency situations. The selected individual will serve as the primary point of contact, offering information, guidance, and advice about our services, as well as facilitating access to social-health services.
Key Responsibilities
* Conduct initial assessments to identify the needs and support requirements of dependent individuals.
* Develop and evaluate support plans, taking into account economic aspects.
* Coordinate with professionals from public social and health services in the area.
* Monitor the roles of caregivers, personal assistants, and home help auxiliaries.
* Participate in the selection and onboarding of Personal Assistants.
* Supervise and follow up on each case, ensuring alignment with our services.
* Promote community engagement in collaboration with local agents.
Requirements
* Degree in Social Work, Social Education, or Psychology (preferred).
* Experience in dependency care and management of teams and services, with strong ties to the specified territories.
* Valid driving license (type B) and own vehicle.
* Residency in Zamora and familiarity with the territory.

Contract Details
We offer two positions: one indefinite full-time contract (38.5 hours/week) with a schedule from Monday to Friday mornings, plus two afternoons to be defined, and one 12-month temporary full-time contract (38.5 hours/week) with a similar schedule.
