RN Care Manager - Full Time - Eddy SeniorCare (Rotterdam)
Eddy SeniorCare has a rewarding opportunity for a full-time RN Care Manager to manage a caseload of patients and help them remain as independent as possible in the community, working with a comprehensive interdisciplinary team.
This position is a community-based HomeCare RN role, providing hands-on nursing care to our participants in their homes.
Work Schedule :
10-hour day shift position, 4 days per week (rotating day off per week)
Requirements :
* NYS RN licensure.
* Two or more years of experience.
* Experience and desire to care for seniors.

About Eddy SeniorCare :
Eddy SeniorCare is an innovative model of care for seniors, located in Latham, 385 Watervliet Shaker Road. It is a Program of All-Inclusive Care for the Elderly (PACE), a unique model that is both the Medicare / Medicaid insurer and healthcare provider, allowing us to customize services to meet participant needs. We care for 140 seniors, age 55 and above, who need long-term support in the community. Our services include a primary care clinic, rehab clinic, licensed home care agency, transportation, adult daycare, home-delivered meals, activities, RN case management, social work, and more. Only 150 PACE programs exist nationwide, making this a truly unique program in the region.
Pay Range : $36.50 - $49.95
Pay is based on experience, skills, and education. Positions exempt under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may vary within the stated range based on location.
