How will you CONTRIBUTE and GROW?
In this role, you will :
* Assist people with diabetes in their training process, ensuring they acquire the necessary knowledge for managing their condition, which should translate into improved self-care and quality of life.
* Train patients in the use of advanced diabetes technologies.
* Gather relevant information related to the training process.
* Follow up with patients to verify proper use of the technologies.
* Continuously update patient records after follow-up.
* Collaborate in the development of diabetes service offerings.
* Once the service offering is implemented, perform broader functions such as patient diabetes education, monitoring their progress, triage, managing alerts from telemonitoring platforms, and applying referral and action protocols as per established guidelines, including verifying compliance with the action plan.
You will be a suitable candidate if you meet the following requirements :
* University Education : DUE / Graduate in Nursing.
* At least four years of professional nursing experience, preferably in diabetes care.
* Dynamic and proactive personality.
* Geographical mobility : coverage area includes Madrid, Castilla la Mancha, and Castilla León (hospitals / centers where we operate). Regular travel is required.
