Professional who will join a rural care team, as the first point of contact with people in a situation of dependency, providing information, guidance and advice on the range of services offered as well as managing new clients.
Duties
* Initial assessment of dependent people, identifying what the person needs and provide the necessary support.
* Draw up the support plan and the financial assessment thereof.
* Coordinate with professionals from the local public health and social services systems.
* Monitoring the work of personal assistants.
* Supervising and monitoring each case.
* Foster a sense of belonging to the community in coordination with local representatives.

Hold a university degree in the field of socio-healthcare and/or complementary services; Social Work, Social Education, DUE, Occupational Therapy...
