Support at Home Care Partner

We are currently seeking an experienced Support at Home Care Partner to join a well-established provider based in Wonthaggi or Leongatha for a 3-6 month full-time contract.

This is a fantastic opportunity for an experienced Case Manager who can hit the ground running, manage a complex caseload confidently, and deliver high-quality outcomes for older Australians living at home.

About the Role

As a Support at Home Care Partner, you will manage a portfolio of Home Care Package / Support at Home clients and ensure services are delivered in line with individual goals, compliance requirements and best-practice standards.

Key Responsibilities

- Manage a caseload of HCP / Support at Home clients
- Conduct comprehensive assessments and regular reviews
- Develop and monitor care plans aligned with client goals
- Coordinate services and liaise with providers
- Ensure budgets are managed effectively and sustainably
- Provide ongoing support, advocacy and communication with clients and families
- Maintain accurate documentation and compliance with aged care standards
- Work collaboratively within a multidisciplinary team

About You

To be successful in this role, you must have:

- Strong experience in Home Care Package (HCP) or Support at Home Case Management
- Ability to manage a caseload independently from day one
- Excellent organisational and time-management skills
- Sound understanding of aged care legislation and compliance requirements
- Strong communication and stakeholder engagement skills
- Relevant qualification in Nursing, Allied Health, Social Work, Community Care or similar (desirable)
- Must have strong experience with completing sign-ups and be conmfortable with completing home visits across the Bass coast region
