Position Overview
All Care is seeking a Speech Therapist to evaluate, diagnose, and treat patients with speech, language, cognitive-communication, and swallowing disorders in a home-based setting. The Speech Therapist works collaboratively with an interdisciplinary team to improve patient function, safety, and quality of life while supporting independence in the home environment.

Work Location: On the road in assigned territory

Essential Duties & Responsibilities
Perform comprehensive speech, language, cognitive, and swallowing evaluations in patients’ homes
Develop and implement individualized treatment plans based on patient needs and goals
Provide therapeutic interventions for communication disorders, cognitive deficits, and dysphagia
Educate patients, families, and caregivers on therapy techniques, home exercises, and safety strategies
Monitor patient progress and modify treatment plans as needed
Collaborate with physicians, nurses, physical therapists, occupational therapists, and other care team members
Recommend adaptive equipment or strategies to enhance communication and swallowing safety
Ensure compliance with home care regulations, policies, and documentation standards
Complete accurate and timely clinical documentation in EMR systems
Participate in case conferences and interdisciplinary team meetings
Qualifications
Valid driver’s license and reliable transportation
Graduate of an accredited speech language pathologist master’s degree program
Active Massachusetts SLP license
One year of experience in a health care settingProficiency in using electronic health record (EHR) systems for documentation and communication.
