We are seeking a person to join our home care team in the Zamora urban area, taking the first contact with individuals in dependency situations, providing information, guidance, and advice about our services, and managing access for new clients to specific social-health services, as well as monitoring current users, always following a person-centered approach.
Responsibilities and Tasks
* Initial assessment of dependent individuals, identifying their needs and support demands.
* Proposing support plans and evaluating their economic aspects.
* Coordinating with professionals from public social and health services in the area.
* Monitoring the roles of caregivers, personal assistants, and home help auxiliaries.
* Participating in the selection and onboarding of Personal Assistants.
* Supervising and following up on each case.
* Aligning services provided by different suppliers.
* Participating in relevant work groups.
* Promoting community engagement in coordination with local agents.
* Degree in Social Work (preferred), Social Education, or Psychology with experience in dependency care.
* Experience in the social-health sector and in managing teams and services, with strong ties to the specified territories.
* Driving license type B and own vehicle.
* Residency in Zamora and familiarity or connection with the territory.
