What you will do:
* Work one-on-one with an assigned patients (full-time or part-time, dayor night shifts are available)
* Ensure quality and safe delivery of home health nursing services.
* Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
* Assist the RN in carrying out the plan of care.
Skills, education, and experience:
* Graduate of accredited practical (vocational) nursing program.
* Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
