Job Title: Specialist, Peer Support
Reports To: Program Manager, Mental Health Substance Abuse
Job Summary: A ssist and guide our behavioral health clients through regular one-on-one support meetings, assist with treatment plans and recovery strategies, coordinate client schedules and logistics, and respond to any emergencies as needed .
Salary Grade : 109
Qualifications and Requirements:

Essential Job Duties:
* Meets with patients for one-on-one support and guidance.
* Assist patients with symptom management and recovery by modeling wellness, personal responsibility, self-advocacy, and hopefulness through the sharing of his or her story.
* Assists patients with developing recovery plans, coping methods, and rehabilitation strategies with patient and behavioral health team.
* Assists, teaches, and supports patients regarding activities of daily living, vocational skills, housing access, maintaining financial support, and building support networks.
* Assists with arranging medical appointments, therapy sessions, support groups, and other resources to aid the patient as indicated by the patient.
* Monitors patient's behavior, schedule, medication, and activities to report back to behavioral health team.
* Liaising with family members and medical professionals regularly.
* Generates recovery reviews and performance documents to track the rehabilitation process.
* Creates and oversees support groups for those in need of group settings for substance abuse treatment and/or specific mental health diagnoses to include scheduling, therapist selection and training, and evaluation.
* Works directly with CCHCI NextGen data team to assist in organization and analysis of data.
* Participates in applicable staff and leadership meetings.

Required Education, Experience, Certificates & Licenses:
* High School Diploma or GED required.
* Completed Peer Support Training with a valid certificate.
* Experience in community engagement, with emphasis on building and maintaining partnerships.
* Any combination of experience and education that provides the required skills may be considered.
* Driver's License and Proof of Insurance may be required if requesting mileage reimbursement.
* Obtain a level one fingerprint clearance card.
Preferred Education, Experience, Certificates & Licenses:
* Experience in program and/or project evaluation, including comfort in use of evaluation tools and methods preferred.
* Personal experience with mental health, substance abuse, and/or trauma preferred.
Required Language Skills:
* Ability to comprehend and compose instructions, correspondence, and communications in English in both oral and written format.
* Ability to effectively present information in one-on-one and small group situations to patients, internal providers and staff and other agency staff working in cooperation with the organization.
Physical Requirements:
* Ability to occasionally exert enough force to move objects weighing up to 10 pounds.
* Ability to continuously remain in a stationary position.
* Ability to constantly move about inside the workplace to assist patients, operate office or medical equipment, etc.
* Possesses hand-eye coordination and manual dexterity necessary to constantly operate computers, telephone, and other office machinery.
* Possesses close visual acuity necessary to accurately record and view information on a computer monitor, handwritten and typed documents.
* Ability to discern the nature of sounds at a normal spoken volume.
Other Required Knowledge, Skills, and Abilities:
* Ability to add, subtract, multiply and divide in all measure, using whole numbers, common fractions and decimals.
* Ability to gather data in an organized fashion from varied sources.
* Ability to perform a variety of assignments requiring considerable independent judgment.
* Ability to deal with challenges involving several variables in routine situations.
* Ability to assess issues / risks and effectively implement remediation measures.
* Computer literacy required.
* Strong community project management skills and the ability to manage multiple high priority projects with good attention to detail.
* Demonstrated ability to oversee students, provide mentorship, develop strategies, and direct student projects.
* Demonstrated ability to collaborate effectively.
Work Environment & Conditions:
* Work environment is typical of an administrative office setting with no substantial exposure to adverse environmental conditions.
* Work occasionally occurs in a health clinic or patient home setting with occasional exposure to communicable diseases, bodily fluids, and hazardous chemicals.
* Work requires reliable transportation as position requires occasional travel and extended hours to include early mornings, evenings, and weekends.
