Position Overview
All Care is seeking a Speech Therapist to evaluate, diagnose, and treat patients with speech, language, cognitive‑communication, and swallowing disorders in a home‑based setting. The Speech Therapist works collaboratively with an interdisciplinary team to improve patient function, safety, and quality of life while supporting independence in the home environment.
Schedule: adaptable
Work Location: On the road in assigned territory
Position Type: Per Diem
Essential Duties & Responsibilities
- Perform comprehensive speech, language, cognitive, and swallowing evaluations in patients’ homes
- Develop and implement individualized treatment plans based on patient needs and goals
- Provide therapeutic interventions for communication disorders, cognitive deficits, and dysphagia
- Educate patients, families, and caregivers on therapy techniques, home exercises, and safety strategies
- Monitor patient progress and modify treatment plans as needed
- Collaborate with physicians, nurses, physical therapists, occupational therapists, and other care team members
- Recommend adaptive equipment or strategies to enhance communication and swallowing safety
- Ensure compliance with home care regulations, policies, and documentation standards
- Complete accurate and timely clinical documentation in EMR systems
- Participate in case conferences and interdisciplinary team meetings
Qualifications
- Valid driver’s license and reliable transportation
- Graduate of an accredited speech language pathologist master’s degree program
- Active Massachusetts SLP license
- One year of experience in a health care setting
- Proficiency in using electronic health record (EHR) systems for documentation and communication
